We do not operate our own data collection systems for patient care. Patient intake forms, health history, appointment booking, and payment processing are handled through AcuBliss, a secure third-party patient portal embedded on our website. We have a Business Associate Agreement (BAA) in place with AcuBliss requiring them to protect your information in compliance with HIPAA. Payments are processed by Stripe, a PCI-compliant payment processor; you can review Stripe's privacy practices at https://stripe.com/privacy.
2. Information Collected
Through the AcuBliss portal, the following types of information may be collected:

• Name, date of birth, and contact details

• Health history, symptoms, and treatment information (Protected Health Information / PHI)

• Credit or debit card details to hold or pay for appointments (processed securely by AcuBliss via Stripe)

• Appointment history and communications with our practice

Through our website directly, we may collect limited non-health information through standard website tools, such as:

• IP address, browser type, device type, and operating system

• Pages visited, time spent on pages, and referring website

• Cookies and similar tracking technologies used by analytics or marketing tools (such as Google Analytics or social media pixels)

• Information you voluntarily submit through contact forms or newsletter signups (such as your name and email address)

This website information is used to understand how visitors use our site, improve our content, and communicate with people who request it. Our website does not independently collect, store, or transmit Protected Health Information — all PHI is handled through AcuBliss.
Important: Please do not include detailed health information in general contact forms, unsecured emails, or text messages. For confidential health communications, please use the AcuBliss patient portal or contact our office directly by phone.

In the event of a breach involving your unsecured PHI, we will notify you as required by the HIPAA Breach Notification Rule.

We reserve the right to update this policy at any time. Changes will be posted on this page with a revised effective date. The updated policy will apply to all PHI we maintain, including records created prior to the change.
